  2. Execute Bill of Sale: Complete this Form MV-912 compliant Bill of Sale with Federal Odometer Certification.
  3. Submit to DMV: The buyer presents this form to New York State Department of Motor Vehicles (DMV) to pay state vehicle sales tax and complete registration.

New York requires Form MV-912 in conjunction with Form DTF-802 (Statement of Transaction - Sale or Gift of Motor Vehicle) to calculate sales tax.
